To support the mission of Helio Health, the Licensed Practical Nurse (LPN) will work in a variety of clinical settings providing medical and counseling services as a member of the multi-disciplinary treatment team. The Licensed Practical Nurse (LPN) will work on a Full-Time basis out of Helio Health’s Elements Residential Program.    The schedule for this position will be Tuesday-Saturday, 3:00pm-11:30pm.

To support the Mission of Helio Health, Inc., the Health Home Care Manager will provide comprehensive care management and coordination of services to Health Home enrollees and eligible Medicaid/Medicaid Managed Care community members.
