Overview

Helio Health is looking for a Targeted Case Manager to support its mission of promoting recovery from the effects of substance use and mental health disorders and other health issues. This role is vital to Case Management and coordination of services to patients/residents participating in, or who have completed our treatment programs. This is a Full-Time position, Monday- Friday 8:00am-4:30pm and located at our CCBHC at West Onondaga St. Program.

 

In addition to Helio Health’s comprehensive benefits package, Full-Time hires for this position are eligible for a Sign-On Incentive up to $1,500!

 

Pay: $20.74 - $26.00 per hour. 

Responsibilities

  • Maintains a case load and performs primary case work responsibilities including:
      • Implementing new strategies on capturing barriers to treatment.  
      • Pulling reports from Healtheconnections and Psyckes, uploading them into EHR. 
      • Meeting with clients to discuss support wrap around services.
      • Assisting clients with obtaining housing, food stamps, transportation, insurance or other immediate crisis needs 
      • Collaborating with care team members to provide appropriate treatment/referrals for clients. 
      • Screening clients to assess their needs. 
      • Represents agency to the community.
      • Other duties as assigned by supervisor. 

Qualifications

  • Associate's degree in related field required, Bachelor's preferred.
  • Qualified Health Professional (QHP) preferred.
  • Two years of experience working with behavioral health and/or substance abuse issues.
  • Valid NYS Driver’s License preferred.
